When you come to your appointment, please remember to obtain and bring the following:
We require referrals to be sent to us for review before an appointment is made. Please email your referral to
- Referral letter from doctor
- Medicare Card/DV card/Pension card
- Details of your private insurance
- Workers Compensation Details
- Any reports, xrays, MRI scans, CT scans etc in your possession.
- Any other information which you feel is relevant
You can also arrange to have the above sent to us prior to your appointment.
Dr Michalka recognises that your time is valuable, and makes every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control.
